Rewards Terms and Conditions

BY ENROLLING IN A LIFE EXTENSION REWARDS PROGRAM ("PROGRAM"), YOU ACKNOWLEDGE AND AGREE TO BE BOUND BY ALL OF THE TERMS AND CONDITIONS ("TERMS") STATED HEREIN. IF YOU DO NOT AGREE TO THESE TERMS, DO NOT ENROLL IN A PROGRAM. POINTS EARNED IN CONNECTION WITH A PROGRAM MAY BE REDEEMED FOR QUALIFYING MERCHANDISE OR SERVICES PURCHASED DIRECTLY FROM LIFE EXTENSION. LIFE EXTENSION RESERVES THE RIGHT TO MODIFY OR TERMINATE THE PROGRAM AT ANY TIME AND IN ANY MANNER, IN WHOLE OR IN PART, AS OUTLINED BELOW, EVEN THOUGH SUCH CHANGES MAY AFFECT THE REDEMPTION VALUE OF POINTS ALREADY ACCUMULATED OR THE AVAILABILITY OF REDEEMABLE MERCHANDISE.

These terms apply to any person enrolled in a Life Extension Rewards Program, including LE Rewards BASIC, LE Rewards PREMIER PLUS, and LE Rewards PREMIER ELITE offered by Life Extension Foundation Buyers Club, Inc. ("Life Extension," "our," "us" or "we").

Offer not available to international customers serviced by distributors of Life Extension® products except as specified at www.LifeExtension.com/Rewards LE Rewards PREMIER PLUS and LE Rewards PREMIER ELITE benefits extend for 12 months from the date of purchase and can be renewed as early as 6 months prior to your expiration date. If renewed, LE Rewards PREMIER PLUS or LE Rewards PREMIER ELITE benefits extend for a full 12 months after your current expiration date, or the renewal date if expired.

1. Privacy

Our Global Privacy Notice available at www.lifeextension.com/legal/global-privacy-notice which are hereby incorporated by reference into these Terms governs the collection, use, transfer, and disclosure of personal information collected under these Terms.

2. Program Eligibility

The Program is open to individuals who are legal residents of their countries of residence and of the fifty (50) United States and the District of Columbia, who are at least eighteen (18) years of age or the legal age of majority in their state of residence, whichever is older, as of the date of enrollment, and have the capacity to enter into a binding contract. Corporations or other entities or organizations of any kind are not eligible for the Program. Employees of Life Extension are not eligible for the Program. The Program is not available to wholesale customers or customers serviced by international distributors, except as specified at www.LifeExtension.com/Rewards.

3. Types of Programs

When you open an account with Life Extension (an "Account") and make a purchase, you will automatically be enrolled as a participant in the LE Rewards BASIC Program and have the option to enroll as a participant in the LE Rewards PREMIER PLUS or LE Rewards PREMIER ELITE Program (each, a " Participant").

The LE Rewards BASIC Program entitles Participants to:

  • A subscription to Life Extension Magazine® if a purchase has been made in the last twelve (12) months;
  • Access to Wellness Specialists—our team of naturopaths, nurses, and nutritionists who will answer your health questions and create a personalized nutritional regimen for you; and
  • Earn points, which accrue at a rate of 2 points per U.S. Dollar spent on each Qualifying Purchase.

The LE Rewards PREMIER PLUS Program entitles Participants to:

  • A 2000-point ($20) product credit upon the purchase or renewal of their LE Rewards PREMIER PLUS Program;
  • A subscription to Life Extension Magazine®;
  • Access to Wellness Specialists; and
  • Earn points, which accrue at a rate of 3 points per U.S. Dollar spent on each Qualifying Purchase.

The LE Rewards PREMIER ELITE Program entitles Participants to:

  • A 5000-point ($50) product credit upon the purchase or renewal of their LE Rewards PREMIER ELITE Program;
  • A subscription to Life Extension Magazine®;
  • Access to Wellness Specialists; and
  • Earn points, which accrue at a rate of 4 points per U.S. Dollar spent on each Qualifying Purchase.

For the purposes of accruing points, a "Qualifying Purchase" includes products, merchandise and lab tests we offer for sale. Shipping fees, CHOICE Shipping Program, LE Rewards PREMIER PLUS or LE Rewards PREMIER ELITE Program fees, Life Extension Magazine® subscriptions, the purchase of gift cards, and any purchase made with points or a gift card are not Qualifying Purchases. For purposes of redeeming points, a Qualifying Purchase is the same as above but also includes routine shipping fees.

4. How to Enroll

You are automatically enrolled in the LE Rewards BASIC Program when you create an Account and make a Qualifying Purchase with us on the Life Extension website, www.LifeExtension.com, in the Life Extension Health & Wellness Market, or by telephone. The online enrollment form requires you to provide your e-mail address and create a password.

You may enroll in the LE Rewards PREMIER PLUS or LE Rewards PREMIER ELITE Programs by signing up on the Life Extension website, www.LifeExtension.com, at the Life Extension Health & Wellness Market, or by telephone with us, and paying the Program fee. Online enrollment requires that you provide your e-mail address and create a password. The LE Rewards PREMIER PLUS Program costs $19.95 and the LE Rewards PREMIER ELITE Program costs $49.95 USD for residents of the United States and varies for residents outside of the United States ordering directly from Life Extension. Fees for LE Rewards PREMIER PLUS or LE Rewards PREMIER ELITE for international customers serviced by specified distributors are listed on the chart available at www.LifeExtension.com/Rewards.

5. How to Earn Rewards Points

Points are calculated as a percentage of the total Qualifying Purchase price. CERTAIN PURCHASES MAY NOT QUALIFY FOR ACCRUAL OF POINTS, INCLUDING BUT NOT LIMITED TO SHIPPING FEES, THE PURCHASE OF CHOICE SHIPPING PROGRAM, LE REWARDS PREMIER PLUS AND LE REWARDS PREMIER ELITE PROGRAM FEES, THE PURCHASE OF GIFT CARDS, AND ANY PURCHASE MADE WITH POINTS OR GIFT CARDS.

Participants may also be eligible to receive bonus Points for specified promotional offers that may be advertised from time to time by Life Extension. The details of any such bonus Points will be provided at the time the offer is made.

The Program structure, including any bonus Points, is subject to modification at any time at Life Extension's sole discretion, including, without limitation, the right to establish additional means of accruing Points, the right to modify and delete any or all of the recognized means of accruing Points, their values and types, and the Points redemption terms, and the right to exclude specific types of transactions from Points eligibility.

Points are accrued after a Participant makes a purchase on www.LifeExtension.com, in the Life Extension Health & Wellness Market, or by telephone, and the product has shipped, and payment has been posted. Only the individual named as the primary Account

Our Returns & Exchanges FAQs available at www.lifeextension.com/returns-faq, which are hereby incorporated by reference into these Terms, outlines the conditions under which products and services purchased from Life Extension may be returned or exchanged, including eligibility criteria, time limits, and any associated fees. For returns, if a Participant has paid with Points, the refund will be provided as Points. For purchases made with both Points and any combination of cash, debit or credit card, or digital payment service (such as Apple Pay, Google Pay, or PayPal), refunds will be credited to the original payment method wherever possible. If the original payment method is unavailable or cannot be processed, store credit or Points will be issued in its place. Refunds are subject to the policies and processing terms of the debit or credit card issuer, bank, or digital payment service used for the original transaction. Participants cannot receive a refund in Points that exceeds the number of Points that were used toward the initial purchase.

A summary of a Participant's Account may be obtained by visiting www.LifeExtension.com and clicking on "My Account" or by emailing customerservice@lifeextension.com. Life Extension will attempt to credit Participants' Accounts with Points on a timely basis. However, each Participant shall have the responsibility of ensuring that his or her Points are properly credited. Any claim for Points not credited accurately must be received by Life Extension within thirty (30) days of the date of claimed accrual of such Points or be barred. Life Extension shall have no liability for any printing, production, typographical, mechanical, or other errors in offering or awarding Points, for any delay or failure to credit Points to Accounts, or for any failure to provide Account summaries as outlined herein. Life Extension reserves the right to invalidate Points from a Participant's Account upon notice if it determines, in its sole discretion, that such Points were improperly credited to such Participant's Account or were obtained fraudulently or otherwise in violation of these Terms. Life Extension reserves the right to require proof of accrual of Points and the right to delay the processing or redemption of any Points without notice, in order to assure compliance with these Terms.

Without limiting any other remedies, Life Extension may suspend or terminate any Account if Life Extension suspects, in its sole discretion, that any Participant or other person has engaged in fraudulent activity in connection with the Program. Points do not constitute property, do not entitle a Participant to a vested right or interest and have no cash value. As such, Points are not redeemable for cash, transferable, or assignable for any reason. The sale, barter, transfer or assignment of any accumulated Points, other than by Life Extension, is strictly prohibited. Any Points that Life Extension deems, in its sole discretion, to have been transferred, sold, bartered or assigned in violation of these Terms may be canceled. Participation in the Program is subject to these Terms, as well as policies and procedures that Life Extension may adopt or modify from time to time. In addition to any other rights or remedies available to Life Extension in law or equity, any failure to abide by the Terms or any policies or procedures implemented by Life Extension, any conduct detrimental to Life Extension, or any misrepresentation or fraudulent activities in connection with this Program, or failure to act in a manner consistent with federal, state, or local laws, regulations, or ordinances, may result in termination of Participant in the Program, as well as forfeiture of any Points accrued and any other benefits earned in connection therewith.

6. How to Redeem Your Points

Once a Participant has accrued any Points in his or her Account, the Participant may apply any or all of the Points toward any Purchase from us. 100 Points earned can be redeemed for $1.00. For example, a Participant who has earned 500 Points ($5.00) may obtain a $5.00 discount on any Qualifying Purchase for redemption from us. One Point earned is equal to $0.01.

No cash will be exchanged for any unused portion of a Participant's Points. Once Points have been redeemed, they are no longer valid for any subsequent redemption and they may not be returned or refunded to a Participant's Account for any reason except for a refund of purchase using Points as addressed in Section 4, paragraph 4.

7. Modification or Termination of Program

The Program is offered at the discretion of Life Extension, and we reserve the right to modify the Terms, Points redemption values, benefits, conditions of participation, rules for issuing, redeeming, retaining, using or forfeiting Points and discounts and their respective duration or timing, or any other aspect of the Program, in whole or in part, at any time, with or without notice, even though such changes may affect the discount value of Points already accumulated. Life Extension will give reasonable advance notice of any modification of the Terms hereof which may adversely impact your Account. The most recent version of the Terms will be available at www.LifeExtension.com. A Participant's continued use of the Program following the posting on Life Extension's website of conspicuous notice of any modification will indicate Participant's acceptance of any modification to the terms hereof. A Participant may object to the modification to the Program and/or its Terms by no longer participating.

Life Extension may terminate the Program at any time in its sole discretion by giving conspicuous notice at least six (6) months prior to the date such notice becomes effective, during which time you may still accrue and redeem Points, provided however, that Life Extension shall not be required to give notice if such termination is due to the filing of a petition in bankruptcy, an adjudication of bankruptcy, insolvency, an assignment for the benefit of creditors, or any other discontinuance of business. You should not rely upon the continued availability of the Program or any Points accumulated in connection therewith.

8. Limitations on Liability and Disclaimer of Warranties

By participating in this Program, you agree that Life Extension and its agents, representatives, affiliates, and employees will have no liability whatsoever for any injuries, losses, claims, damages or any special, exemplary, punitive, indirect, incidental or consequential damages of any kind, whether based in contract, tort, strict liability or otherwise, resulting from any use of the Program, any failure or delay by Life Extension in connection with the Program, or the performance or non-performance of the Program by Life Extension, even if Life Extension has been advised of the possibility of damages. This disclaimer of liability applies to any damages or injury caused by any failure of performance, error, omission, interruption, deletion, defect, delay in operation, computer virus, communications line failure, theft or destruction, or unauthorized access to, alteration of, or use of your registration information. Notwithstanding this disclaimer, if Life Extension is found liable for any loss or damage which arises out of, or is in any way connected with, any of the occurrences described in this paragraph, then its liability will in no event exceed, in total, the sum of $100.00.

ife Extension makes no warranty of any kind regarding the Program, which is provided on an "as is" and "as available" basis. Life Extension expressly disclaims all warranties, including implied warranties of merchantability, fitness for a particular purpose, title, non-infringement, and those arising by statute or otherwise in law or from a course of dealing or usage of trade. Life Extension is not responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to the Program, including without limitation that the Program will be error-free, or as to the accuracy, completeness and timeliness of any content or information distributed with respect to the Program. SOME STATES DO NOT ALLOW THE LIMITATION OF LIABILITY AND DISCLAIMER OF IMPLIED WARRANTIES, SO THE DISCLAIMERS AND LIMITATIONS ABOVE MAY NOT APPLY TO YOU.

9. Additional Terms

The failure of Life Extension to comply with the Terms because of an act of God, war, fire, riot, hurricane, flood, terrorism, earthquake, actions of federal, state, provincial or local governmental authorities or for any other reason beyond the reasonable control of Life Extension shall not be deemed a breach of the Terms. If any such contingency shall last for more than sixty (60) days, Life Extension shall have the right to terminate the Program immediately by posting notice on www.LifeExtension.com and/or by email to Participants and Life Extension shall have no further liability to any Participant.

These Terms make up the entire agreement between Life Extension and Participants relating to the Program and replace any prior understandings or agreements (whether oral or written) regarding the Program. The laws of the State of Florida without regard to its conflict of law principles will govern these Terms. If you take any legal action relating to your use of this Program or these Terms, you agree to file such action only in the state or federal courts located in Broward County, Florida.
