A) No. As part of your test price, we provide an order from a licensed doctor in your state through our network.
A) All ages can be tested. However, we do require a signed parental consent form for anyone under the age of 18.
A) Yes. Laboratory tests are not available for purchase in Maryland. Also, the states of NY, NJ, RI, MA and PA have state laws governing laboratory testing. See FAQs below.
A) No. Life Extension is not a medical facility and we do not have a doctor/patient relationship with you. Therefore, we cannot diagnose, prescribe or treat, and no specific medical advice will be given. We offer Laboratory testing for informational purposes only.
A) No. All laboratory testing must be privately paid for at the time of purchase. We accept credit/debit cards and checks. We do not bill Medicare, Medicaid or private insurance companies. Also, we are unable to provide ICD-10 or CPT codes or our Tax ID number.
A) No. We only offer laboratory tests to people residing in the continental US and in Anchorage, AK.
A) We encourage everyone to use their own doctor and health insurance for blood tests if possible. But many customers say it is easier and more convenient for them to use us instead of taking time out of their busy schedules to make appointments and wait around in a doctor's waiting room. Our customers also mention their shock at how much they have to pay if their insurance won't cover the tests compared to what they would pay through us. Also we offer unique blood panels you won't find elsewhere, plus your own doctor may not be willing to write a prescription for blood tests for preventive purposes.
Conventional Lab Testing FAQs
A) We work with Laboratory Corporation of America (LabCorp), the world's leading healthcare diagnostics company. Your Laboratory tests will be analyzed by them.
A) It takes time for orders to be reviewed, processed, and approved. Once you place your order, you will receive your LabCorp Requisition within 24 hours if you have chosen email or fax as the method of delivery (allow up to 48 hours for orders placed on Saturday). If you choose to receive your requisition by mail, or are receiving a blood draw kit, please allow up to 5 business days for delivery. All requisitions and kits are mailed from Ft. Lauderdale, Florida.
A) Most people will have access to thousands of LabCorp patient service centers nationwide!

A) Yes!! Although we send an electronic version of your order directly to LabCorp it is still necessary to bring a paper copy to avoid any delays in your testing process.
A) Yes. Your requisition form is good for 6 months from the date it was ordered. However, Life Extension can extend the expiration date an additional 6 months at your request. Just call customer service at 1-800-678-8989. If you have received a blood draw kit, expiration is based on the tubes included. You can call for a new kit if your tubes expire before the kit is used.
A) Yes. State restrictions apply for residents of NY, NJ, RI, and MA. People residing in these states will receive a blood draw kit. The kits cannot be taken to LabCorp; however, most local hospitals can provide blood draws on an out-patient basis. Extra fees for processing and packing your specimens may apply and are your responsibility.
A) If you live in NY, NJ, RI, or MA, you will receive a prescription for the blood draw signed by a doctor in our network. You will also receive all of the necessary blood draw tubes, instructions, and a pre-paid shipping label. Please note that most of the samples will need to be centrifuged prior to shipment, so you will need to find a facility that can draw, process, and pack the blood for you.
A)It depends on the test and how you choose to receive results. Results from Life Extension National Diagnostics, Inc. can be sent to you by USPS first class mail, e-mail, or fax. You also have the convenient option to view your results online, through your web account, or send them to your health care provider. Results will usually arrive within 5–7 business days from the date of the blood draw, depending on the complexity of the laboratory tests ordered. In many cases results come back much faster!
A) Your lab results will look like a standard lab report. Your personal healthcare provider can discuss them with you, or you can call a Life Extension Wellness Specialist free of charge to review your results at 1-800-226-2370.

A) No. All of Life Extension National Diagnostics Inc. special tests are conveniently packaged as kits which you will use to collect your specimens in the comfort of your home.
A) No. All of Life Extension National Diagnostics Inc. special tests include a pre-paid shipping label for either UPS, USPS, or FedEx.
A) Yes. A requisition will be packaged with your kit.
A) All kits are shipped to you via USPS first class mail. You can choose expedited shipping via UPS for an extra fee.
A) It depends on the test and how you choose to receive results. Results from Life Extension National Diagnostics, Inc., can be sent to you by USPS first class mail, e-mail or fax. The choice is yours. Results of specialty tests cannot be viewed online. Results will usually arrive within 3 weeks from the date the kit arrives at the testing laboratory.
A) Yes. Kits cannot be shipped to a Pennsylvania address due to state laws and some additional restrictions may apply in the state of New York.
A) No. We only offer specialty tests to only people residing in the continental US.
