Evaluates total and free testosterone along with SHBG levels
Testosterone is a steroid hormone from the androgen group primarily secreted in the testes of males and the ovaries of females with small amounts also secreted by the adrenal glands. This testosterone test uses gold-standard methodology that can quantify very low (and very high) testosterone values.
This test measures total and free testosterone, as well as SHBG (sex hormone binding globulin).
Total Testosterone (LC/MS-MS)
Testosterone by liquid chromatography and tandem mass spectrometry (LC/MS-MS) provides accurate measurements at very low concentrations, quantifying results of 2 ng/dL or greater, and so is the suggested test option for those with very low testosterone levels. This test can also be used for measuring levels above 1,500 ng/dL, as it provides an assay measurement range up to 2,000 (and up to 10,000 with dilution procedures) (https://www.cdc.gov/clinical-standardization-programs/media/pdfs/2024/04/CDC-Certified-Testosterone-Assays-508.pdf)
Free Testosterone by Equilibrium Dialysis
Free testosterone is measured by a methodology called equilibrium dialysis.
Free (unbound) testosterone diffuses across a semi-permeable membrane until equilibrium is reached. At equilibrium, the amount of diffused testosterone is then measured to determine the free teststoerone concentration.
SHBG (Sex Hormone Binding Globulin)
SHBG is now included with our Testosterone Free/Total test. SHBG is a binding protein that plays an important role in maintaining healthy hormone levels.
Testosterone and estradiol circulate in the bloodstream, bound mostly to sex hormone binding globulin (SHBG) and to some degree other blood proteins. Only a small fraction of the sex hormones are unbound, or "free," and thus biologically active and able to enter a cell and activate its receptor.
SHBG decreases the active form of the hormone available to the tissues. Thus, bioavailability of sex hormones is influenced by the level of SHBG. High levels of insulin and androgens decrease SHBG level. On the other hand, thyroid hormone, and estrogen increase it.
Instructions
Fasting is not required for this test. Take all medications as prescribed. If you are supplementing with any hormones, we suggest taking them approximately 2 hours prior to having your blood drawn to see peak levels. Hormones are best drawn between 8-10 AM.
